    Services include:

    • Enhanced Care Management (ECM): intensive, hands-on support for individuals with complex needs, including care coordination, health system navigation, and advocacy.
    • Community Supports (CS): Flexible, health-related services to address social drivers of health, such as Housing Transition and Navigation, Housing Tenancy and Sustaining, and Connections to other Community Supports like medically tailored meals, recuperative care, respite services, and more through health plans.

  • Hours: Mon: 8am - 5pm; Tue: 8am - 5pm; Wed: 8am - 5pm; Thu: 8am - 5pm; Fri: 8am - 5pm; - (Office is closed every 3rd Friday for team meetings. Office is closed on New Year's Day, Martin Luther King Jr's Birthday, Lincoln's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Veterans Day, Thanksgiving Day and Christmas Day.)

    Program Delivery
