    Offers additional services to Medi-Cal members with complex needs, helping with both health and personal challenges to improve long-term well-being.

    A Lead Care Manager works with doctors, specialists, mental health providers, and other care staff to ensure patients receive the care they need. A Lead Care Manager will meet the patient at their preferred location.

    Lead Care Manager helps with:

    • Coordinating care based on the patient's specific medical and social needs
    • Patients understand and manage medications, secure refills and arrange pharmacy deliveries
    • Schedule appointments
    • Connect Patients to community resources such as food, housing, transportation, and other needs

    Program Delivery

    • Eligibility: Meet eligibility requirements: -Be a Medi-Cal member with Health Plan of San Joaquin, HealthNet, or Kaiser Permanente -Live in the state of California And meet one of the following requirements: -Experiencing homelessness or at risk of becoming homeless -Someone who frequently needs to visit the emergency room or hospital because of an ongoing health -Living with serious mental health or substance use disorder needs -A current or former child or youth involved in the child welfare system -A pregnant or postpartum individuals (up to one year after giving birth) who is Black, American Indian, Alaska Native, or Pacific Islander -An adult with complex needs who need a high level of support in their life ranging from health to social services
