    Provides home-delivered meals under the CalAIM Community Supports program to those with qualifying chronic conditions. As part of the Medi-Cal Manage Care Benefit, program delivers fully prepared, medically tailored meals directly to the home.

    Meals are locally prepared, and Nutrition Based. Culturally Diverse Health Meal Options and Variety of Menu Options available. Offers 2 meals a day, 7 days a week up to 12 weeks.

    Program Delivery

    • Eligibility: Must meet program guidelines: -Must be an active member of a Medi-Cal HealthNet (Coming soon Medi-Cal Health Plan of San Joaquin) -Must have a chronic health condition or one of the following: -Gestational diabetes and other high-risk pregnancy conditions -Chronic or disabling mental or behavioral health disorders -Patients discharging from the hospital or skilled nursing facility -High risk of hospitalization or nursing home placement -Care needs that require multiple doctors
