Meals are locally prepared and nutrition-based, with culturally diverse health meal options and a variety of menus. The service offers 2 meals per day for up to 12 weeks.

    Program Delivery

    • Eligibility: Meet program guidelines: -Must be an active member of a Medi-Cal HealthNet or Health Plan of San Joaquin -Must have a chronic health condition or one of the following: -Gestational diabetes and other high-risk pregnancy conditions -Chronic or disabling mental or behavioral health disorders -Patients discharging from the hospital or skilled nursing facility -High risk of hospitalization or nursing home placement -Care needs that require multiple doctors
