    Program Delivery

    • Eligibility: Must meet eligibility criteria to be able to get a referral: *Please do not submit referrals with any disqualifying factors *Must be a released or discharged veteran who served a minimum of 180 days of active military services *Must HAVE a valid DD-214 form (Certificate of Release or Discharged from Active Duty) *Must NOT have a dishonorable discharge from military service *Must NOT have a felony conviction *Must NOT be listed or have any sex offender registry For rental eviction prevention: *Meet referral eligibility criteria and *Must receive an eviction or pay or quit notice for unpaid rent For mortgage assistance: *Meet referral eligibility criteria and *Must receive a foreclosure documentation or default notice on mortgage loan For rent deposit: *Meet referral eligibility criteria and *Must be experiencing homelessness

    • Application Process: By referral only. 211 Call Specialist can complete referral form to veterans who qualify. Veteran must have a working phone number on referral form. Turnaround time takes seven days of the initial referral for intake case manager to contact.
