CalAIM Community Supports - Mom's Meals Home Delivered Nutrition Program

    Provides home-delivered meals under the CalAIM Community Supports program to those with qualifying chronic conditions. As part of the Medi-Cal Manage Care benefit, program delivers fully prepared, medically tailored meals directly to the home.

    Meals are packaged for convenient storage in the fridge for up to 14 days from delivery. Offers nine condition-specific menus:

    • Heart Friendly
    • Lower Sodium
    • Protein+
    • Pureed
    • Renal Friendly
    • Vegetarian

    Program Delivery

    • Eligibility: Must meet program guidelines: -Must be an active member of a Medi-Cal health plan -Must have a chronic health condition or one of the following: -Gestational diabetes and other high-risk pregnancy conditions -Chronic or disabling mental or behavioral health disorders -Patients discharging from the hospital or skilled nursing facility -High risk of hospitalization or nursing home placement -Care needs that require multiple doctors
    • Languages: English and Spanish as well as translation services to support over 160 languages
