Digital Vaccine Record (DVR)

  • Description: Provides a statewide online portal for individuals to obtain a digital copy of their vaccine record. A link to a QR code will be provided to access the vaccination record.

    If a parent or guardian have multiple vaccine records associated with a single cell phone number or email address, enter each Digital Vaccine Record request separately.
