Agency: CHILDREN'S HOME OF STOCKTON

  • Description:

Provides a shared room for youth experiencing or at-risk of homelessness.

Transition Age Youth (TAY) participants work with coaches to build independent life skills, get guidance to reach goals, and access center, education, and mental health support. Meals and transportation, like bus passes or Uber rides, are provided as needed.

TAY Housing Navigator helps TAY participants plan housing transitions and prepare for aftercare support upon successful program graduation.
