Disaster Case Management Program (DCMP)

  • Description: Helps storm disaster survivor develop a realistic long-term recovery plan and identify and facilitate access to appropriate community resources.

    Service includes:
    • General Case Management
    • Recovery Assistance
    • FEMA Appeals
    • Connection to support system
    May not be able to assists with challenges experienced before disaster, direct mental health services, legal advice, personal errands, co-sign and personal transportation. No guarantee of housing and financial assistance.

    Program Delivery

    • Eligibility: Must have a disaster-related need and resided or formerly resided in San Joaquin County during Winter 2022 and Spring 2023.
    • Languages: English, Spanish
    • Application Process: Telephone call, call to schedule an appointment, or apply online at www.ccstockton.org. Sonora office by appoinment only.
