    Program Delivery

    • Eligibility: Individuals aged 18 and over in need of non-emergency medical transportation. Individuals under aged 18 must be accompanied by an adult.
    • Languages: English only
    • Application Process: Call to schedule an appointment. 7 days advance notice preferred.
    • Program Fees: Call for details. Accepts Health Plan of San Joaquin, Humana. Sliding scale if no insurance.
    • Required Documents: Photo ID and Medical Insurance Card for those insured
