Program coordinates care services based on the individual's medical, functional and social needs. Wrap around Health Plan includes medical, dental, vision, transportation to and from medical appointments, medication, long-term care insurance and more.

    Program Delivery

    • Eligibility: Must meet criteria:
      *55 years of age or older
      *Receive Medi-Cal or Medicare
      *Have stable housing and meet nursing level criteria - Able to live safely at home and in the community without support
      *Lives in the WelbeHealth service area: San Joaquin County
    • Languages: Other languages through interpreter services, Hmong, Spanish, English, Chinese
    • Application Process: Call for telephone intake
    • Program Fees: Call for information; Accepts Medi-Cal, Medicare
    • Required Documents: Photo ID and Insurance Card
