    • Treatment of chronic health conditions such as hypertension, diabetes, and asthma
    • Treatment of Pediatrics cystic fibrosis, hemophilia, cerebral palsy and heart disease
    • Treatment of Pediatrics asthma, ADHD and diabetes
    • Referral to in house Behavioral Health for stress, anxiety, and depression

    Program Delivery

    • Eligibility: Open to all Medi-cal, Medicare, or private medical insurance recipient and those uninsured.
    • Languages: Other languages available upon request, English, Spanish
    • Application Process: Call to schedule an appointment
    • Program Fees: Accept Medi-cal, Medicare, and most private medical insurance plans. Sliding scale fee for those uninsured.
    • Required Documents: CA ID, medical insurance card for those insured, verification of income or last year tax return for the scaling fee program
