Tracy Homeless Case Management

  • Description: Provides for a duration of three months a time case management, information and referrals service to those experiencing homelessness or at risk of becoming homeless.

    Assistance includes:
    • Employment assistance
    • Housing referrals
    • Obtaining an ID
    • Help apply for CalWORKs, CalFresh, Medi-Cal, Social Security Retirement Income (SSA), Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI)
    • Connects to Emergency Shelters, Food (Soup Kitchen), Medical, Dental, Mental Health Services, and Homeless Recuperative Care
