  • Hours: Mon: 7am - 5pm; Tue: 7am - 5pm; Wed: 7am - 5pm; Thu: 7am - 5pm; Fri: 7am - 5pm; - (Office closed on New Year's Day, Martin Luther King Jr's Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.)

    Program Delivery

    • Eligibility: Individuals aged 18 and over in need of nonemergency medical transportation. Individuals under aged 18 must be accompanied by an adult.
    • Languages: English and Spanish
    • Application Process: Call to schedule an appointment. Reservations should be made with 1-2 week notice.
    • Program Fees: No fees
    • Required Documents: Full name, phone number, pick up time, appointment time, pick up address and drop off address. Also provide information if patient needs a return ride and if children will be riding.
