    Program Delivery

    • Eligibility: Must be 18 years of age or older to ride alone. Individuals under the age of 18 must be accompany by an adult.
    • Languages: Spanish, English
    • Application Process: Door-to-Door Service: Call to schedule an appointment. Reservations should be made with 1-2 week notice.
    • Program Fees: Free
    • Required Documents: Door-to-Door Transportation: Must provide patient's full name, phone number, pick up time, appointment time, pick up address and drop off address. Also provide information if patient needs a return ride and if children will be riding.
