    • Eligibility: Must meet qualification criteria: *Must be 18 years of age or older *Must be of Native American heritage *Must meet household income below Federal poverty level *Must live in service area

    • Application Process: Call for telephone intake or complete NIDC Intake Form via website
    • Program Fees: No fees
    • Required Documents: Photo ID, Social Security Card, Tribal Verification, Income Verification of six-month period, and Proof of residency
