  • Description: Provides an online database with information about retirement, disability, survivor, Medicare, and Supplemental Security Income claims and appeals.  Individuals can request a replacement Social Security card, request a Social Security Statement, or get a benefit verification letter.  Individuals that are receiving benefits or have Medicare can change their address and phone number, start of change direct deposit of the benefit payment, get a replacement Medicare card, and get a replace SSA-1099 or SSA-1042S for tax season.
