    Program Delivery

    • Eligibility: Must meet one of more of the following criteria:

      *Must be 18 years of age or older
      *Provides in-home care to an individual 60 years of age or older and frail
      *A grandparent 55 years of age or older caring for a grandchild under the age of 18
