    Program Delivery

    • Eligibility: Must meet ALL of the following criteria:

      *Must be 18 years of age or older
      *Not receiving services from Department of Developmental Services or Department of Rehabilitation
      *Not receiving case management under the Department of Mental Health or Multi-purpose Senior Services Program
      *At risk of being homeless
    • Languages: Spanish, English
    • Application Process: Call for telephone intake or to schedule an appointment
