  • Description: Provides in-home services to patients and families facing terminal illnesses. Hospice team includes a medical director, nurses, certified nursing assistants, physical, speech, and occupational therapists, social workers, spiritual advisers, and volunteers.

    Program Delivery

    • Eligibility: Must be 18 years of age, diagnosed with advanced chronic and/or life limiting illness and under a physician care, and have less than 6 months of living.
    • Languages: Other languages available through the language line, English, Spanish
    • Application Process: Telephone call
    • Program Fees: Sliding. Accepts Medicare, Medi-Cal and All Managed Health Care. Call for Information.
    • Required Documents: Medical Insurance Card for those insured.
