  • Description: Provides non-medical in-home care services to older adults and Veterans to stay in their homes. Works with individuals with dementia and Alzheimer.

    Services include:
    • Personal Care Services – Assists with bathing, hygiene, dressing, toileting, and physical therapy
    • Light Housekeeping
    • Companion Care – Provides emotional well being and mental health support and activities
    • Medication Reminder
    • Meal Preparation – Plan and pre meals, grocery shop and clean-up
    • Transportation – Provides door-to-door transportation to and from destination.
    • Alzheimer’s and Dementia Care
    • Care Management – Geriatric Care Managers monitor, advocates, and provide care to love ones

    • Program Fees: Sliding. Accepts Private Pay, Long Term Care Insurance, Veteran's Benefits
