    • Companion Care – Provides emotional well being and mental health support and activities
    • Medication Reminder
    • Meal Preparation – Plan and pre meals, grocery shop and clean-up
    • Transportation – Provides door-to-door transportation to and from destination.
    • Alzheimer’s and Dementia Care
    • Care Management – Geriatric Care Managers monitor, advocates, and provide care to love ones

    • Eligibility: Older adults, Veterans, and spouse of Veterans aged 65 and older requiring assistance.
