Community Health Care Program (CHCP)

Provides health coverage for qualifying California residents who don't have access to other health coverage. Plan includes benefits such as preventive care, medication, and pediatric dental coverage and vision services up to age 19.
CHCP members:
- Are enrolled in the Kaiser Permanente Platinum 90 HMO plan
- Pay no monthly premium
- Pay no of pocket costs for most covered services at Kaiser Permanente facilities

Program Delivery
- Eligibility: Must meet requirements: -Live in a Kaiser Permanente service area that is open to new enrollment -Household income is no more that 3 times the federal poverty level -Do not have access to any other health coverage, including Medi-Cal, Medicare, an affordable job-based health plan, or coverage through Covered California -In most cases, child dependents must be younger than 26. -Individuals over 21 are eligible to receive the CHCP subsidy for a maximum of up to 3 years in a row. Eligible dependents can continue to receive the subsidy until they turn 21, no matter how long they’ve been in the program. -The CHCP subsidy is available as a one-time resource, and all members must reapply annually. Members who do not reapply in consecutive years will lose future eligibility.
