  • Description: Arranges free air transportation via commercial airlines so that patient may have access to specialized out-of-state medical care and second opinions throughout the United States.  Arranges air travel for parents or legal guardians to travel with child patient.

    Air transportation can be arrange to attend service dog trainings as required by service dog organizations.

    Program Delivery

    • Eligibility: Patient Criteria -Under the age of 17: up to 2 parents/legal guardians -Age 18 and above: 1 medically necessary caregiver -Flights are subject to income eligibility with supporting documentation -Qualifying applicants will receive assistance as often as needed

    • Required Documents: Proof of income, proof of relationship of parent to child patient, and proof of medical need and letter of appointment at treatment site
