Hospice Care

Provides 24-hour residential nursing care and symptom management, dietary support, meals, laundry, activities and amenities of home when patients are no longer able to remain at home. Medical equipment such as hospital beds, oxygen and bedside commodes are provided.

Program Delivery
- Eligibility: Must have a life expectancy of six months or less (based on their physician’s prognosis) or Answer yes to four of following questions: *have disease or injury that cannot be cured *spend most of the day in bed or in a chair *have to take pills for very bad physical, body pain *have made a lot of trips to the emergency room in the past 6 months *doctor informed that my life expectancy is limited *need help from other people with daily activities like taking a shower, getting dressed, eating, cooking, walking, or getting out of bed *started feeling more tired and weak *lost a lot of weight in the last six months
