Program Delivery

• Eligibility: Must have a life expectancy of six months or less (based on their physician’s prognosis) or Answer yes to four of following questions: *have disease or injury that cannot be cured *spend most of the day in bed or in a chair *have to take pills for very bad physical, body pain *have made a lot of trips to the emergency room in the past 6 months *doctor informed that my life expectancy is limited *need help from other people with daily activities like taking a shower, getting dressed, eating, cooking, walking, or getting out of bed *started feeling more tired and weak *lost a lot of weight in the last six months
• Languages: Call for other languages offered, English
• Application Process: Call for more information or to make a referral.
• Program Fees: Hospice services and medications related to the terminal diagnosis are covered 100% by Medicare and Medi-Cal. For those who are not a Medicare/Medi-Cal beneficiary, hospice care is a benefit covered by most private insurers.
