    Program Delivery

    • Eligibility: Open to all seniors 60 years of age or older. Case Management Eligibility Requirements: 路 Age 60 years or older 路 Experiencing difficulties that put one at risk of living independently 路 Be unable to manage personal affairs due to emotional and/or cognitive impairment 路 Impairment in one or more areas of Activities of Daily Living or two or more Instrumental Activities of Daily Living 路 Currently not receiving services from an outside agency 路 Participants must be able to continue to live independently 路 Reside in San Joaquin County
