  • Description: Provides 24-hour residential nursing care and symptom management of the terminal illness.

    Services include comfort medications, medical supplies, medical equipment, provide teaching for family on how to provide care for the patient, and respite care for the caregivers.

    Care is provided at the patient’s home, a skilled nursing facility, residential care home (assisted living facility), or at Hospice of San Joaquin Residential Living Center.

    Program Delivery

    • Eligibility: Must be at least 18 years of age and have a limited life expectancy (based on a physician’s prognosis), have completed and/or no longer receive therapies and no longer seeking curable treatment.
    • Languages: Other languages available through the language line, English, Spanish

    • Program Fees: Sliding; Accepts Medicare, Medi-Cal; Private Insurance, HMO and private pay.
