    Program Delivery

    • Eligibility: Must meet eligibility criteria:
      -Must be a single individual with permanent disability
      -Experiencing homelessness such as living in an emergency shelter (or similar location) or a place not meant for human habitation (car, street, tent, abandon building)
      -Be homeless continuously for the past 12 months or had at least 4 separate homeless periods in the past 3 years adding up at least 12 months
      -Meet income guidelines
      -Be able to share a housing environment and care for self
      -Must have proof of residency in San Joaquin County
    • Languages: Other language available through the language line, English, Spanish
    • Application Process: Telephone call. By referral only through Coordinated Entry. Screening and intake is available through 211 Monday-Friday 9am-4pm.
