    Provides permanent supportive housing assistance for individuals with disabilities who are experiencing homelessness and their partner. Central Valley Housing does not provide housing. Eligible household can search for housing throughout San Joaquin County. The cost of housing must meet US Department of Housing and Urban Development (HUD) guidelines.

Office is closed on New Year's Day, Martin Luther King Jr's Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.)

    Program Delivery

    • Eligibility: Individuals with a permanent disabling condition who have been homeless for 12 consecutive months in a shelter or a place not meant for human habitation, or who have had four separate homeless periods totaling 12 months in the past three years, and who meet income guidelines and are willing to develop a self-sufficiency plan and live in San Joaquin County.
    • Languages: English and Spanish
    • Application Process: By referral from Coordinated Entry System (CES). Call 211 San Joaquin for CES screening and intake Mon-Fri 9am-4pm.
    • Program Fees: No fees
    • Required Documents: Proof of residency in San Joaquin County, documentation of homelessness and disability, verification of income, and photo IDs and social security cards for both members.
