    Offers home-based, outreach, and consultant support to ease the discomfort, symptoms, and stress of serious illness.

    Service includes:

    • Assistance and guidance to improve quality of life
    • Open discussion about treatment choices, including disease treatment and symptom management
    • Coordinate care with health care providers
    • Emotional, physical, psychological, and spiritual support
    • Nursing: In-home visits, 24-hour phone support, medication education, and information on disease and disease process
    • Social Services: Help with community resources, in-home visits, and emotional support
    • Chaplain Services: Spiritual support and in-home visits

    Palliative care works alongside treatment providers and is not a replacement for primary treatment.

    Program Delivery

    • Eligibility: Individuals with serious illnesses experiencing pain or other symptoms, needing help understanding their condition, discussing treatment, and requiring advocacy and support services.
    • Languages: English and Spanish. Additional languages are provided via a translation service.
