    Program Delivery

    • Eligibility: Must meet eligibility critieria:

      -Children from birth to 21 years of age
      -Lives in San Joaquin County
      -Has a qualifying conditions such as:
      *Cerebral Palsy
      *Spina Bifida
      *Muscular Dystorphy
      *Rheumotoid Arthritis
      *Spinal Cord Injuries
      *Arthrogryposis
      *Osteogenesis Imperfecta
      *Head Injuries
    • Languages: Phone translation available for other languages, English
    • Application Process: By referral
    • Program Fees: Free; Sliding scale base on income
