    Provides physical and occupational therapy services to eligible patients at no cost. Services conduct in outpatient clinics located in designated public schools.

    Service includes:

    • Attendance at school meetings such as an Individualized Education Plan (IEPs).
    • Home and school site visits to assess specific equipment needs.
    • Evaluation and custom fitting of durable medical equipment, such as wheelchairs
    • Medical Therapy Conferences (Clinic) to provide a team approach to child's special needs at no cost.

  • Hours: Mon: 8am - 5pm; Tue: 8am - 5pm; Wed: 8am - 5pm; Thu: 8am - 5pm; Fri: 8am - 5pm; - (Office closed on New Year's Day, Martin Luther King Jr's Birthday, Lincoln’s Birthday, Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Veteran's Day, Thanksgiving Day and the day after, Christmas Eve, Christmas Day, and New Year’s Eve.)

    Program Delivery

    • Eligibility: Must be under 21 years of age who have a CCS eligible medical condition and reside in San Joaquin County. Qualifying conditions such as: • Cerebral palsy • Spina bifida • Muscular dystrophy • Rheumatoid arthritis • Spinal cord injuries • Arthrogryposis • Osteogenesis imperfecta • Head injuries There is a financial eligibility requirement for specialized equipment. *Family annual income must be under $40,000
    • Languages: Additional languages are provided via a translation service, English, Spanish
    • Application Process: Call for assistance or by referral
    • Program Fees: Call for fee information; Accepts Medi-Cal
    • Required Documents: Medi-Cal card and current medical documentation of medically eligible condition. Non Medi-Cal applicants: Social Security Card, Driver's License, rent and/or utility receipts/bills, check stubs, health insurance information, and State and Federal Income Taxes.
