Serves homeless veterans and those at risk of becoming homeless by providing wrap-around supportive services such as: rental assistance, utility payments, household items, child care, transportation, medical/mental health referrals, affordable health care, housing navigation, money management, food/nutrition assistance and much more.

    Program Delivery

    • Eligibility: Veteran must meet all of the eligibility: *Must be without a home or at risk of being without a home *Must meet income requirements *Must have served in the United States military or a member of a family in which the head of the household or the spouse of the household head is a Veteran *Received a General or Honorable discharge and meet minimum Active Duty requirement if in the Guard or Reserve

    • Required Documents: Picture ID, Social Security Card, VHA card, DD214 form, verification of all income, lease or housing documentation, and utilities shut off notice, eviction notices. Complete HUD packet if applying for HUD-VASH Housing voucher
