    Program Delivery

    • Eligibility: Must be under 21 years of age who have a CCS eligible medical condition and reside in San Joaquin County. Qualifying conditions such as: • congenital heart disease • cancers, tumors • hemophilia, sickle cell anemia • thyroid problems, diabetes • serious chronic kidney problems • liver or intestine diseases • cleft lip/palate, spina bifida • hearing loss, cataracts • cerebral palsy, uncontrolled seizures • rheumatoid arthritis, muscular dystrophy • AIDS • severe head, brain, or spinal cord injuries, severe burns • problems caused by premature birth • severely crooked teeth • broken bones There is a financial eligibility requirement for specialized equipment. *Family annual income must be under $40,000

    • Program Fees: Free for full scope no share of cost for Medi-Cal; For all coverage, call for information
    • Required Documents: Medi-Cal card and current medical documentation of medically eligible condition. Non Medi-Cal applicants: Social Security Card, Driver's License, rent and/or utility receipts/bills, check stubs, health insurance information, and State and Federal Income Taxes.
