    Provides a preventive program that delivered periodic health assessments and services to low-income children and youth in California. Provides care coordination to assist families with medical appointments scheduling, transportation, and access to diagnostic and treatment services. 

    Health assessments were provided by enrolled private physicians, local health departments, community clinics, managed care plans, and some local school districts.

    Program Delivery

    • Eligibility: Open to California residents under 21 years of age with Medi-Cal benefits or Non Medi-Cal recipient under 20 years of age with family income at or below 200 percent of the federal income guidelines. And children in Foster Care.
