    Provides non-medical support to seniors and individuals with disabilities, helping them stay independent at home and avoid unnecessary nursing home placements.

    Multipurpose Senior Services Program (MSSP) is a voluntary program that focuses on encouraging frail, elderly residents to make health lifestyle choices for their social, health, and housing needs.

    Care management includes:

    • comprehensive health and psychosocial assessment
    • care planning with the client and family
    • arrangement of services
    • coordination of care with other community agencies advocacy
    • some limited payment of in home services
    • ongoing monitoring
    • annual reassessments
    • quarterly home visits and monthly contacts

  • Hours: Mon: 8:30am - 4:30pm; Tue: 8:30am - 4:30pm; Wed: 8:30am - 4:30pm; Thu: 8:30am - 4:30pm; Fri: 8:30am - 4:30pm; - (Closed Mon-Fri 12noon-12:30pm and on New Year's Day, Martin Luther King Jr's Birthday, President's Day, Good Friday, Easter Monday, Memorial Day, Independence Day, Labor Day, Veteran's Day, Thanksgiving Day and the day after, Christmas Eve, and Christmas Day.)

    Program Delivery

    • Eligibility: Current client, aged 65 and older, homebound with health issue like fall risk or institutionalization risk, and have Medi-Cal without share costs.
