  • Hours: Mon: 8am - 5pm; Tue: 8am - 5pm; Wed: 8am - 5pm; Thu: 8am - 5pm; Fri: 8am - 5pm; - (Office is closed on New Year's Day, Martin Luther King Jr's Birthday, President's Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Veteran's Day, Thanksgiving Day and day after Thanksgiving, Christmas Eve, Christmas Day, and New Year's Eve)

    Program Delivery

    • Eligibility: Children and youth from birth to 18 years of age (up to 20, if still in school) and families. Must have full-scope Medi-Cal
    • Languages: English and Spanish. Additional languages are provided via a translation service.
    • Application Process: Call for assistance
    • Program Fees: No fees. Accepts Medi-Cal
    • Required Documents: Photo ID, income verification, children's Medi-Cal card, and proof of custody arrangement or legal guardianship of child
