Enhanced Care Management

Offers care management services to Medi-Cal Kaiser members with complex medical and social needs, supporting care coordination among doctors, specialists, pharmacists, social service providers, and members, and helping find and apply for community services.
Services include:
- Regular doctor visits for preventative care when sick
- Specialty Care Visits
- Help with housing or shelter
- Behavioral health and mental health services
- Substance use disorder treatment
- Developmental health
- Oral health (dental)
- Long-term services and supports (LTSS)
- Referrals to community resources

- Outreach and engagement
- Comprehensive assessment and care management
- Enhanced coordination of care
- Health promotion
- Comprehensive transitional care
- Member and family support services

Program Delivery
- Eligibility: Active Medi-Cal Kaiser member of all ages and one of the following requirements: *Homeless, at risk of losing housing, or fleeing domestic violence *Frequent ER visits or hospital stays *Serious mental illness *Substance use disorder *Recently released from jail or prison within the year *In a nursing facility, or at risk of needing one *Intellectual or developmental disability *Pregnant or recently postpartum within the year *Children and youth in child welfare, California Child Services (CCS), or experiencing homelessness
- Languages: English and Spanish. Additional languages are provided via a translation service.
