    Program Delivery

    • Eligibility: Active Medi-Cal Health Net member and one of the following requirements: *Homeless, at risk of losing housing, or fleeing domestic violence *Frequent ER visits or hospital stays *Serious mental illness *Substance use disorder *Recently released from jail or prison *In a nursing facility, or at risk of needing one *Intellectual or developmental disability *Pregnant or recently postpartum *Children and youth in child welfare, California Child Services (CCS), or experiencing homelessness
    • Languages: English and Spanish. Additional languages are provided via a translation service.
