    Program Delivery

    • Eligibility: Must be at least 18 years of age, able to sit and no bed bound.

    • Application Process: Call for telephone intake or call to schedule an appointment. 7 days advance notice preferred
    • Program Fees: Accepts Health Plan of San Joaquin, Humana; No insurance: scaling price based on mileage. Call for other health insurance
